Two examples spring to mind:
1. PBeM match, Manhandlers vs Sylvan City Blades. A Blitzer throws a double-skulls block, re-rolls into another double skulls, falls down, gets broken. AV-1. No problem, I have an Apothecary! She botches horribly.
2. Underworld Creepers (Undead) vs Baskerville Blood-Donors (Vampire). I end up throwing a double-down/skull combo each of my last four turns as the first action of the turn with my MA4 Mummy. And he used my last re-roll in the first of those turns, so make it 5 in a row to end the game. I had ball in hand only to get LBd and blitzed THREE TIMES in a 4-1 loss.
I later avenged that loss with a massive Pitch Invasion in overtime of the Spike final for that year

The minotuar on my chaos dwarf team, The Prison Rodeo, is famous in my league for his terrible rolls of block dice.
By far his worst sting of rolls came in a game against my friends orcs:
On the first turn of the game I'm recieving, he blocks and gets a block-skull and a skull. He doesn't have block and can't use rerolls so down he goes causing a turnover on my first action.
He spends the next turn standing up and then proceeds in turn 3 to perform a repeat of turn 1. Spends the next turn standing up and guess what in turn 5 he goes down again.
Both my opponent and I a rolling on the floor with laughter at this point and my hobgobbo is hiding in the corner with the ball to keep it out of play as long as possible.
I'll bet you can guess what happens in turn 7. Not only does Mr. Mino knock himself down for the 4th consecutive block, he also suffers a serious injury and is now AV7.

I had my worst luck against a Dwarf team with my WE team.
This Dwarf team had lost all his games in the league ( not a good Dwarf coach ). I started to feel the wrath of those nastie players with Tackle soon enough. My LineElf with Guard, Block, Dodge died in the 1st block made against me. My Wardancer with +1 AG and +1 ST died in turn 3
Well, lets get to the unlucky part, the above is just usual with WE teams. It was 5 turns left of the game in second half. He kicked to me and the score was 2-1 to me. I had a pretty good control of the match, could score now in 2 turns and lead 3-1. My Thrower went down to pick up the ball, yes a 1, sure hands, 1 again. I cursed my bad luck but knew i had time to make it up. Turnover and the Dwarfs came down for the ball, went slow. My next turn i try to pick the ball up again, double 1 again. The opposing coach is laughing his ass off. Turnover and he gets even closer, but is one square from my thrower and the ball. I try to pick up again, DOUBLE 1 AGAIN. Thats 6 one's in a row. Turnover, he picks up the ball with a dice roll of 6 and dodge from my thrower with a dice roll of 5 and scores a TD.
2 turns left of the game, score 2-2. Ofc i can score. I picked up the ball this time, and it was time to run on the sides. Only thing left now was for my Catcher with most SPPs to run and make one 2+ dodge. He fails on the dodge and the reroll, through the armour, and yes, he DIED. The dwarf coach so happy for his 1st game not losing, i had 3 dead and 4 SI. Well thats the life for any Elf coach.
I retired the Thrower and made him an assistant coach. And every time some misses the pickup, they shout, YOU PULLED AN OLE(my name) and laugh at him.

A friend had pretty awful luck over the w/e. Playing a game (not BB) and 6 consequitive die rolls he gets the worst possible result. That's about 45,000-1 odds. The worst part is we'd be playing the game for about 12 hours at that point (and less than half way through) and had to stop as it meant he'd lost the game

In the TBBF my Goblin team Da Maggot Scum are notorious for their strange luck, most bad.
For example: I have literally had one of the Ogres Bonehead six times in a row, in the end he Boneheaded TWELVE frackin' times that game! At least the other one only did it three times. (I wrote it off as a side-effect of the team getting a new sponcership deal from Guinness.)
In a tournament match, I've Triple Skulled, TRR, Tripled again. Then for
insult to injury one of the Ogres goes and Triple Skulls on the first block the next turn, sigh...
Then there was time one Ogre spiked THE SAME Goblin four times in a row on a Throw Team-mate play. K'o'ed him all four times, on the FIFTH attempt, that Goblin player I set the Hell away from him and instead put a Crippled Rookie, oh gee look perfect throw now.
Though I must admit, the more impossible the play is, the more often my Goblins somehow pull it off. It's the easy parts they flub.
Maybe I should stop using StarFleet Battles Dice.
Well at least I never lack for opponents in the League.

My Chaos Warrior fails a GFI. He then fails the re-roll after I use a TRR. He then gets his armor penetrated. He then suffers a casualty. He then dies. Then my apothecary fails. That's six consecutive 1/6ths against me.
On top of that, in the same game, 3 out of about 15 2-dice blocks of mine were double-skulls.
I had one game against an Undead team, and luck was so slanted in his favor that at the end he dodged with his mummy and picked up the ball. At least the mummy was AG2 though. I got him back a couple minutes ago though, when two of our different teams were playing, and 2 PI's were rolled in a row (my FF 10 vs his FF 2...).
I think I was probably most frustrated with the lost my team suffered against a Dwarf team. The guy didn't start with any runners, but it wound up not mattering because throughout the game he dodged his longbeards into TZs, would pick up the ball in TZs, and then dodge away again, and GFI twice, never needing a re-roll.
And of course, any game involving elves, you might as well assume as a loss before you start, since they invariably roll long strings of 1's and skulls.

My worst streak of bad luck have to be one time i played with orcs.
I kept rolling double-skull on the dices every single time i blocked with my blackorcs.
Not even once did they manage to pushback/knockdown an enemy player!
Another time i had bad luck, again with orcs, was against dark elf were i failed most of my armour rolls while the dark elf succeded with most of their armour rolls.
And then it was that time with wood elf were i kept failing dodge rolls and dodge rerolls even when i only needed 2+.

a rel lucky or unlucky story (depence for which coach):
We have a Skaven Coach (lucky one) who tryed a desesperate blitz with a gunter runner against an Ogre (from Trambi's Ogre Team)...
he put him down, passed his armor, killed him with Sigurd Table....
And Trambi (the unlucky coach of the story) rolled a '1' with his Apothicary...
Always be aware of a gunter runner!!!!

Yes any player can be hazardous. In our league we have a helf team that has a player named Suicide( i believe the current one is the 3rd), the helf linemans (without dauntless) whole job is to do stupid blocks like that, thing is the original wracked up a decent number of casualties.
